Choices for Youth is proud to offer a variety of programs and services to help young people live healthier, more stable lives. Below is a list of referral forms for you to refer a young person, or to self-refer to a program. All referral forms are in PDF format, and once completed should be faxed to the appropriate number indicated on the form.

PROGRAM REFERRAL FORMS
housing
The Lilly is a 14 unit, congregate living housing model for youth that works. Both the programming and the physical layout of the building promote a sense of community, while maintaining individual home spaces in a youth-friendly environment.

The RallyForward program provides at-risk youth and emerging adults access to affordable housing (16 supportive housing units spread over multiple homes in the St. John's area), paired with a system of intensive case management and wrap-around supports.

The Soft Landing program operates from a Housing First model and aims to provide a rapid rehousing and shelter diversion approach for youth who are precariously housed, experiencing homelessness, or experiencing transitional homelessness between the ages of 16 and 29. The program offers a one stop entry into housing, by offering a coordinated approach, meaning on referral to access Housing and supports within CFY's housing and support programs.

As an accessible, safe, and immediate option to begin work towards greater stability, The Choices Shelter is an essential service in our community, providing a safety net for young people who find themselves in extremely difficult and stressful circumstances.

The Education, Employment and Social Enterprise (EESE) department acts as the hub for all pre-employment, employment, and educational supports for young people at Choices for Youth.

The Momma Moments Program is a peer-to-peer support and strengths-based group for young parents or pregnant mothers. It provides moms and their children with wrap-around support to navigate the challenges of motherhood.
